Winter in Japan can be very dry. Many visitors and new residents find that their lips feel rough or start to crack. The good news: every drugstore has a wall of lip balms, and many cost less than ¥1,000.
The hard part is the label. Most boxes are only in Japanese. Some balms are cosmetics, some are quasi-drugs, and some are real medicines. This guide explains the labels in simple English. Then it shows 7 popular lip care products sold in Japan, with their Japanese names, ingredients, sizes and prices on 1 November 2026. We also cover tax-free shopping, allergies and when to see a doctor.
We did not test these products. The information comes from the makers’ official pages and large drugstore chains’ online stores. There is no ranking here; the order follows the type of product.
📌 What you will learn
In a hurry? Japanese drugstores sell three kinds of lip care. 化粧品 (cosmetic) balms add moisture. 医薬部外品 (quasi-drug) balms help prevent rough and cracked lips. 第3類医薬品 (Class 3 OTC medicine) treats lips that are already cracked or sore. Prices start at about ¥107. If a medicine does not help after 5–6 days, stop and talk to a doctor or pharmacist.

Cosmetic, quasi-drug or medicine? Read the box
Japanese law puts lip care products into different groups. The group is printed on the box. Look for these words:
| Label on the box | What it means | Use it for |
|---|---|---|
| 化粧品 (keshōhin) | Cosmetic. No “active ingredient” is listed. | Daily moisture and a nice feel |
| 医薬部外品 (iyaku-bugaihin) / 薬用 | Quasi-drug. Has an active ingredient (有効成分) with a mild effect. | Preventing rough, dry and cracked lips |
| 第3類医薬品 (dai-san-rui iyakuhin) | Class 3 over-the-counter medicine | Treating lips that are already cracked or sore |
※ Based on PMDA and 日本OTC医薬品協会 (JSMI) explanations, checked 1 November 2026.
医薬部外品 (quasi-drug)
Japan’s medicine law says a quasi-drug has a mild effect on the body (「人体に対する作用が緩和なもの」). PMDA, the national drug agency, explains it like this: a quasi-drug is not a medicine. Its main purpose is prevention, not treatment. It is made for everyday use, without a doctor or pharmacist watching. Many quasi-drug balms say 薬用 (medicated) on the front.
The difference from a cosmetic is the active ingredient. A quasi-drug lists 有効成分 on the box. On a lip balm, the claim is usually 「唇の荒れ・ひび割れを防ぐ」, which means “prevents rough and cracked lips”.
第3類医薬品 (Class 3 OTC medicine)
This is a real medicine that you can buy without a prescription. Japan has three classes of over-the-counter medicine. The Japan Self-Medication Industry (日本OTC医薬品協会) explains that Class 3 covers all OTC medicines that are not Class 1 or Class 2. Class 1 needs a pharmacist. For Class 3, the law does not require staff to explain the product. You can pick it up from the shelf in most drugstores.
Class 3 lip medicines list conditions such as 口唇のひびわれ (cracked lips), 口角炎 (sore corners of the mouth) and 口唇炎 (inflamed lips). Everyday balms do not make these claims.
✅ Japanese words to look for
- 無香料 = unscented
- 無着色 = no added colour
- メントール / カンフル = menthol / camphor (cooling feel)
- 有効成分 = active ingredient
- 効能・効果 = what it treats or prevents
- SPF / PA = sun protection (UVB / UVA)
How to choose a lip balm for winter in Japan
- How bad are your lips? For dry lips, a cosmetic or quasi-drug balm is fine. If your lips are already cracked or sore, look at the 第3類医薬品 products.
- Do you need sun protection? For skiing or long days outside, a balm with SPF helps. Two of our picks show SPF on the box. Reapply often.
- Scent and cooling. Choose 無香料 if you do not like scent. Menthol and camphor give a cool, fresh feel. Some people like it, others do not.
- Size. Our sticks range from 1.5 g to 4.2 g. The medicine tubes are 7–8 g.
- Ingredients you avoid. Some balms contain lanolin, honey, royal jelly or parabens. Read the full ingredient list (成分) on the box.

- If redness, swelling, itching or irritation appears during use or after sun, stop and see a dermatologist (皮膚科)

Tax-free shopping, allergies and halal
Can I buy lip balm tax-free?
Yes, but only if you spend enough. Japan’s tax-free system changed on 1 November 2026. It is now a refund system. You pay the full price with tax in the shop. When you leave Japan, customs checks that the goods are leaving. Then the shop, or its refund provider, pays the tax back.
Before, cosmetics and medicines were “consumables” (消耗品). They needed at least ¥5,000 before tax and a special sealed bag. Under the new system, the split between general goods and consumables is abolished, and the sealed bags are gone. The minimum is still ¥5,000 (before tax) per shop per day.
A lip balm costs ¥107 to ¥1,320, so one stick will not reach ¥5,000. You would need to buy other items in the same shop on the same day.
✅ Tax-free checklist for drugstore shopping
- Spend at least ¥5,000 (before tax) in one shop on one day
- Bring your passport when you buy
- Do not use the products in Japan. Goods used in Japan are not eligible. If you used some, tell the customs officer
- See customs within 90 days of purchase, before you check in your bags
- Keep every item on that receipt. If one item is missing, no item on that receipt is tax-free
- Ask the shop how and when it pays the refund. Methods and fees can differ
Tip: if you need a balm right now, buy one separately, without tax-free. Keep the tax-free items packed for the trip home. Read more in our Japan tax-free shopping guide.
Allergies and sensitive lips
Read the ingredient list (成分) on the box. From the lists we checked:
- Lanolin: メンソレータム 薬用リップスティック (refined lanolin) and DHC 薬用リップクリーム (liquid lanolin)
- Honey / royal jelly: ニベア ディープモイスチャーリップ; honey also in メルティクリームリップ
- Paraben and l-menthol: モアリップN
- Fragrance: メンソレータム 薬用リップスティック
“Allergy-tested” (アレルギーテスト済み) does not mean that no one will react. Try a small amount first. If you have a known allergy, ask a pharmacist or your doctor.
Halal and vegan
We did not find a halal or vegan statement from any of these makers. Some products contain animal-derived ingredients such as lanolin, honey or royal jelly. Please check halal certification and the full ingredient list yourself, or contact the maker.
Frequently asked questions
What is the difference between 医薬部外品 and 第3類医薬品?
医薬部外品 (quasi-drug) has a mild effect and is mainly for prevention, such as preventing rough and cracked lips. 第3類医薬品 is an over-the-counter medicine. Lip medicines in this class list treatments for cracked lips, 口角炎 and 口唇炎.
Which lip balm is best for already cracked lips?
Look for a 第3類医薬品, such as 資生堂 モアリップN or メンソレータム メディカルリップ ヒビプロ. Their labels list cracked lips. If there is no improvement after 5–6 days, stop and talk to a doctor or pharmacist.
Which lip balms have sun protection?
Among our picks, メンソレータム メルティクリームリップ has SPF25 / PA+++. ニベア ディープモイスチャーリップ shows SPF26 / PA++ on NIVEA’s page; one shop listing shows SPF20, so check the box. Reapply often.
Which ones are unscented?
Look for 無香料. The Melty Cream Lip and NIVEA Deep Moisture Lip have 無香料 versions. Curel Lip Care Cream and Moilip N are unscented. The Mentholatum Medicated Lip Stick contains fragrance and has a cool menthol feel.
Can I buy lip balm tax-free?
Yes, if you are an eligible visitor and spend at least ¥5,000 (before tax) in one shop on one day. From 1 November 2026, you pay tax first and get it back after customs checks the goods. Products you use in Japan are not eligible.
Are these lip balms halal or vegan?
We found no halal or vegan statement from the makers. Some contain lanolin, honey or royal jelly. Please check certification and ingredients yourself.
The short version
For daily care in dry Japanese winters, a cheap stick like メンソレータム 薬用リップスティック (¥107) or an unscented balm like キュレル リップケア クリーム (¥935) works well. For sun protection, choose メルティクリームリップ (SPF25) or ニベア ディープモイスチャーリップ. If your lips are already cracked, use a 第3類医薬品 such as モアリップN or メディカルリップ ヒビプロ (both ¥1,320). If they do not get better in 5–6 days, see a doctor or pharmacist. For tax-free, spend ¥5,000 before tax in one shop, and do not use the items before you leave Japan.
